Transitional care managers keep patients on the road to recovery, help prevent readmissions

Yale-New Haven Hospital's transitional care managers (TCMs) are registered nurses with care management experience whose roles extend beyond the hospital, into patients' homes and the community.

The main goal of the hospital's Transitional Care Manager Program is to help patients continue their recovery after discharge and to prevent hospital readmissions. Reducing readmissions is one of YNHH's 2015 Performance Incentive Plan (PIP) safety and quality goals.

New devices provide on-demand video interpreting

Take an Apple iPad. Attach it to a metal pole with wheels. Roll it into a clinical unit. What do you have? An on-demand, video remote interpreting device that improves the lines of communication between healthcare provider and patient and enhances the patient experience.

The devices, dubbed "iPoles" (interpreter poles) by the Interpreter & Translation Services Department, are part of a new project being implemented throughout YNHH, including off-site locations. The pads on poles, along with larger devices that use a 23-inch touchscreen instead of an iPad, will replace the Martti carts used previously for video interpreting, said Maureen Rosselli, deaf and hard of hearing services coordinator.
