YNHH partners with community shelter to provide respite care

Yale-New Haven Hospital is among the supporters of the new Medical Respite Care program at Columbus House, a community shelter in New Haven. The program includes a newly renovated 12-bed unit where medically fragile patients can be transferred upon discharge from YNHH. At Columbus House, they are cared for by professional and medical staff including visiting nurses and a patient navigator.
