Employees will choose their 2014 benefits during open enrollment Monday, Oct. 28 through Friday, Nov. 15 online by going to www.ynhhsbenefits.com.
All benefits-eligible employees must go online and enroll or their medical coverage will default with a "tobacco usage" surcharge of $20 per person per pay period.
For dental coverage, 2014 is an even plan year so enrollment is open and elections will be in place for 2014 and 2015; premiums have not increased from 2012.
All employees must certify that their dependents continue to be eligible for healthcare coverage in 2014. While enrolling for benefits, employees should also verify that their life insurance beneficiaries for group term life are correct. Employees must enroll online at work or home or by using one of the kiosks located around the YNHH campus. Employees who need assistance with online enrollment should call the Benefits Office at 688-2401.
At the benefits fair, employees can get more information from representatives from YNHH Benefits Office, Anthem, CVS Caremark, Delta Dental, Vision Service Plan, Crosby Benefit Systems, Fidelity Investments, Marsh Voluntary Benefits, ActiveHealth, YNHH Employee and Family Resources Program, the Lifeline Program and the YNHH Fitness Center who will be on hand to answer questions.
Employees will receive a personalized open enrollment kit at their homes in advance of the fair days which are:
Tuesday, Nov. 5: Medical Center South: 10:30 a.m.-noon Shoreline Medical Center: 1-4 p.m.
Wednesday, Nov. 6: Hawley Lane Office, Trumbull: 8:30-10:30 a.m. YNHH Campus, 2nd floor Atrium: 11:30 a.m.-4 p.m.
Thursday, Nov. 7: YNHH Campus, 2nd floor Atrium: 7:30 a.m.-2 p.m.
Friday, Nov. 8: Saint Raphael Campus, Allspice Café: 10 a.m.-4 p.m.
Monday, Nov. 11: Grimes Center Recreation Room: 9-11 a.m.
To enroll, all benefits-eligible employees must go online and elect their benefits for 2014 by Nov. 15.
