Advance Directives

Sometimes your particular condition or situation may prevent you from voicing your decisions, beliefs and preferences about your medical care. Having an advance directive can help.

The Choice is Yours

As a patient, you have the right to make your own healthcare decisions, guide your medical treatment and plan what should be done in the event that you are unable to speak for yourself. Advance directives are legal documents (also known as living wills) designed to inform your loved ones and healthcare providers about your healthcare preferences.

Federal law requires hospitals to give you information about advance directives, as well as to honor any such document that is completed and filed with the hospital in which you are receiving care.

How We Can Help

Our team can help you create your own advance directives. We will provide and instruct you on how to complete and submit the forms necessary to make your healthcare preferences known ahead of time, or to name someone who can make these decisions on your behalf.

Once you file your signed advance directives, it is our job to ensure your preferences are respected and adhered to by our medical staff.
