If you have been discharged from the hospital and would like to see your record, please submit your request to the Yale-New Haven Hospital Health Information Management Department (address below).
If you would like a copy of your medical records sent to your doctor or other healthcare provider, please complete the "Authorization for Access/Release of Health Information" form. Please use the form, as you must submit your request in writing.
If someone other than the patient is making the request to access the medical record, the patient's signature or signature of the patient's legal guardian or representative is required.
