Diabetes Healthcare Program
If you or your loved has diabetes, you may attend the four-week Diabetes Healthcare Program. Our program is recognized by the American Diabetes Association as meeting the National Standards for Diabetes Self-Management Education. It consists of four (4) two and a half hour (2½) hour sessions covering a variety of topics related to diabetes self-management. While there is a fee, the cost is coverd by most medical insurance plans. The classes are offered Wednesday (9 - 11:30 am) or Tuesdays (3 - 5:30 pm) for four consecutive weeks. In addition to the four standard classes, individual sessions to learn a specific skill (such as insulin administration or blood glucose monitoring) can also be arranged.
To enroll, call 203.867.5695 with your name, address, phone number and the name of your physician. A physician referral is required. Check with your health insurance company regarding fees for diabetes education.

This clinic is available every other week and serves as a diagnostic and consultation resource for patients with endocrine problems, including diabetes.
