Family Health Center

The Family Health Center at Yale-New Haven Hospital Saint Raphael Campus offers a range of primary health care services and medical specialty services.

The Family Health Center provides comprehensive primary health care to patients throughout their lifecycle. Services include:

  • Anticipatory guidance, developmental screening, and health education
  • Chronic disease management
  • Collaboration with community-based providers, including rehabilitation services, specialty care providers, and home health agencies
  • Coordination of the continuum of care, including referrals to specialists, admissions for inpatient services, and the establishment of post-discharge treatment plans
  • Diagnosis and treatment of acute and episodic illnesses
  • Prenatal and women's health services
  • Primary care (prevention) services
  • Well-baby and well-child care
  • Preventive dental care for children ages one to 6

The Family Health Center's clinical team includes: internal medicine physicians, pediatricians, obstetricians/gynecologists, nurse practitioners, certified nurse-midwives, physician associates, pharmacists, and physicians who provide a wide range of medical specialties including: pulmonology, endocrine/metabolic, renal (kidney), and rheumatology (joint and muscular system disorders).

Insurance and finanacial assistance

Our primary care clinicians participate in Connecticut's four HUSKY A Medicaid managed care plans, as well as fee-for-service Medicaid, and the SAGA program. Financial counselors provide assistance to uninsured patients about various publicly- funded insurance programs and other sources of coverage. Our patients have access to a number of supportive services, such as social work, diabetic education, a medication assistance programs, nutritionist, WIC, and family literacy promotion.

Within the Ambulatory (Outpatient) Surgical Specialty Clinics on the second floor of the Orchard Medical Building (330 Orchard Street), patients have access to a variety of specialty services. The clinical team includes general surgeons, orthopedic surgeons, urologists, vascular surgeons, podiatrists, otolaryngologists and plastic surgeons.

Adult Primary Care

Adult primary care services include:

  • Anticoagulation Service (blood thinning - heart attack/stroke prevention)
  • Lung disorders/Pulmonary services
  • Metabolic Service (diet, nutrition)
  • Renal (kidney)
  • Rheumatology (joints and muscle pain)

Obstetrics and Gynecology

We are an experienced, multi-disciplinary team that strives to improve the quality of life of our patients. We are committed to providing exceptional care; preserving the dignity and well-being of each individual; and serving our community. We work together to provide confidential, affordable healthcare to all women and children. The same clinician will see the patient throughout her pregnancy and will continue her postpartum and well-woman care. Emphasis is on education and coordination of care.

Gynecological services

  • Well-women care in all phases of life

  • HIV counseling and testing
  • WIC referrals
  • Teen gynecological care (for patients 17 and under)

Individuals may face emotional, psychological, social and financial challenges. Our social workers provide a full range of services to support women and children.

Financial assistance

Pediatric Primary Care

We provide pediatric primary care, newborn to age 18. We also provide pediatric neurology care, pediatric development and behavioral care and asthma care.

Social work

Individuals may face emotional, psychological, social and financial challenges. Our social workers provide a full range of services to support women and children.

Financial assistance

Cost is based on a sliding scale. Reduced and no-cost services are available to those who qualify. A financial counselor is available. We participate in HUSKY, Healthy Start, Title XIX and SAGA.
