If you need a copy of your child's medical record, you must complete an "Authorization for Access/Release of Information" form. To obtain the form, call the medical information unit at 203-688-2231 during business hours. Return the completed form to Yale-New Haven Hospital, Clinical Information Services, Medical Information Unit, 20 York Street, New Haven, CT 06510-3202; or by fax: 203-688-4645. If copies are sent directly to you, there is a per-page copying fee. If copies are sent directly to a physician or hospital, there is no charge.
