Transitional Care Center

Matthew Esposito, 79, has been coming to the Transitional Care Center at Yale New Haven Hospital for the past two years. He receives diuretics to reduce fluid buildup caused by congestive heart failure.

“The Transitional Care Center team is terrific,” said Esposito. “They’re very knowledgeable, and are available to help me and my wife with my healthcare needs. There are no long waiting times for phone calls or appointments. When I need to speak with a nurse, someone is always available. The door is open for me to be seen whenever necessary. Carey or Dionne will call me. My wife and I really like that. The follow up is very good in this area.”
