Deadlines are quickly approaching for employees to participate in the Know Your Numbers program and qualify for a $500 credit on their 2014 health insurance premiums.
Know Your Numbers is designed to help employees heighten their health awareness, improve their health status and save money. Benefits-eligible employees who undergo confidential health screenings and complete an online health assessment questionnaire will receive a $500 credit toward the cost of their 2014 YNHHS medical premium.
The Know Your Numbers program has two steps: a health screening and an online health assessment. Both have to be completed to be eligible for the $500 credit.
Due to high employee response to onsite screenings, one final event has been scheduled on the York Street Campus for Wednesday, August 21 from 7 a.m.-4 p.m.
Employees can go to www.pickatime.com/ynhhs.htm to schedule an appointment and walk-ins will also be accepted. Employees who have had an annual physical since May 1, 2012 may also ask their physician to fill out the provider form with their screening results (available on the Employee Self Service dashboard). The completed form must be mailed or faxed to Impact Health no later than Friday, August 23.
"Employee response to the health screenings has been positive," said Michael Dimenstein, vice president, Compensation and Benefits, YNHHS. "It would be a shame if any employees miss their opportunity to save on next year's insurance premium cost by missing this important deadline."
Step 2 of Know Your Numbers is completing an online health assessment. In order to give employees ample time to complete the health assessment step, the deadline has been extended until Sunday, September 15. When employees see the word "Congratulations" on the "My Rewards" page, they will know they have successfully completed both steps of the Know Your Numbers process.
Employees can access the online health assessment by logging onto myactivehealth.com/ynhhs. The numbers from their health screening should have automatically uploaded into their health assessment. If not, employees should complete the assessment and fill in the lab values they received at their screening.
If employees have trouble completing the assessment, they should call the Active Health Customer Service line at 866-938-0320. Employees who do not have access to a computer may stop by the Benefits office on the York Street campus or the Human Resources office on Selina Lewis 3 on the Saint Raphael Campus to complete the health assessment.
Participation in the Know Your Numbers program is voluntary and employees' privacy is protected. YNHHS is working with third-party vendors to conduct these programs and will only receive an overall summary of the health screening and assessment results — no individual employees' results will be shared with YNHHS.
