Geriatrics (Aging)

Overview

Our Geriatric team provides a range of services including: outpatient specialty geriatric evaluations through the Dorothy Adler Geriatric Assessment Center; comprehensive outpatient primary care through the Home Based Primary Care program and traditional office based primary care; a dedicated inpatient and emergency department consultation service; the Center for Restorative Care for Older Adults, a 22-bed unit on the Saint Raphael campus; and post-acute care at the Grimes Center, a 112-bed short-term rehabilitation facility.

Geriatrics is medical care dedicated to treating older adults. A geriatrician — a physician specializing in the medical care of older adults — has a full understanding of the wide range of physical, mental, medical, social and spiritual issues that older adults can have. A geriatrician is trained to identify how these problems can combine to limit an older person’s ability to enjoy life. With expert treatment, older adults can live the fullest life possible.

Geriatric Programs and Services

Center for Restorative Care for Older Adults

The Center for Restorative Care (CRC) specializes in providing acute inpatient care for patients who are 65 and older. The CRC seeks to minimize the hazards of hospitalization for older adults who are receiving treatment for acute medical issues by employing a multidisciplinary team that focuses on patient priorities, mobility, delirium prevention, medication management and maintaining patients' independence in the community.

Nurses Improving Care for Healthsystem Elders (NICHE)

NICHE is a nationwide program that provides resources to hospitals to improve the care delivered to all older adults. It specializes in providing patient-centered geriatric care and connecting patients with appropriate community resources when necessary.

The following Geriatric Vital Signs are reviewed daily:

  • Cognition
  • Mobility
  • Nutrition
  • Skin issues
  • Sleep problems

The CRC team of medical providers is comprised of physicians fellowship trained in Geriatric Medicine, nurses, physical and occupational therapists, social workers, pharmacists, nutritionists and care coordinators. These medical providers ensure that patients maximize their function and cognition throughout their hospital stay.

Dorothy Adler Geriatric Assessment Center

Our team of experts meets with the patient’s family members or caregivers to determine how medical, psychological, cognitive or social problems are impeding his or her function or threatening independent living. We work closely with the patient and the family, as well as, the patient's physicians, and other care providers to develop a comprehensive plan to optimize function, independence and quality of life.

Evaluations include:

  • review of medical history and multiple comorbidities
  • assessments of mobility, mood, and memory
  • review of prescribed and over-the-counter medications
  • discussions of the patient’s goals and priorities

We also help patients and families develop connections to appropriate community services such as home health care agencies, adult daycare centers and volunteer support groups. In conjunction with the patient’s other clinicians, our team provides ongoing case management and clinical care. Furthermore, our center serves as an important educational site for medical residents, fellows, and nurse practitioners in geriatric medicine.

Geriatric Home Based Primary Care

This team of specially trained geriatricians, nurse practitioners, licensed practical nurses (LPNs) and geriatric care coordinators provides in-home primary care for homebound older patients with the goal of enabling these patients to remain at home. A physician on the Home Based Primary Care team will become the patient’s primary care provider and the team will work with the patient and his or her family to create an individualized plan of care to address the individual’s specific healthcare needs.

Geriatric Inpatient Consultation

If geriatric expertise is needed while your loved one is in the hospital, one of our geriatricians can conduct a focused geriatric assessment. To request a consultation, please ask your inpatient care team to place a referral for an inpatient consultation.

Geriatric Primary Care

Older adults face a unique set of medical issues and their medical and emotional needs change with age. Our geriatric specialists offer primary care specifically focusing on these issues. We work with patients to maximize their mobility and function; screen and treat mood and cognitive changes; reduce the number of prescribed medications; and most importantly, we tailor our care to address our patient’s goals and priorities.

Emergency Department Consultation

A dedicated nurse practitioner provides geriatric consultative care at the request of emergency department physicians at both the York Street and Saint Raphael Campuses to help manage older patient’s needs in this acute setting.

Grimes Center

The Grimes Center is a skilled nursing facility conveniently located adjacent to the Yale New Haven Hospital Saint Raphael Campus.  As part of Yale New Haven Health, the Center has full access to all of the system resources and expertise.
