Great Catch winners help YNHH become a high reliability organization

Congratulations to these Great Catch winners for modeling HRO safety behaviors and taking action to improve patient safety.
Inpatient
Lynda Sams-Harris, radiology technologist, Diagnostic Imaging, was recognized for attention to detail while performing an exam. Noticing rib fractures and a white area in the patient’s lung, Sams-Harris escalated her concerns. The patient was upgraded to a modified trauma status, and additional testing showed life-threatening injuries. Sams-Harris’ use of CHAMP behaviors prevented a negative outcome.
Outpatient
Lynn Thompson, RN, Women’s Center, was honored for attention to detail and practicing a questioning attitude while providing pregnancy test counseling to a patient with diabetes. Noting the patient’s history of elevated lab test results, Thompson escalated her concerns to the attending physician and checked the patient’s blood sugar. When it was found to be extremely elevated, the patient was sent to the ED and subsequently admitted.
