Elizabeth Sammarco, RPh 

Elizabeth Sammarco, RPh, clinical pharmacist, earned the Great Catch Award for identifying a discrepancy between a patient’s reported lisinopril dose and the documented prescription history. Sammarco engaged the patient in a detailed discussion to clarify the discrepancy, reviewing his prescription patterns across multiple states and encouraging the patient to call his wife, who had the current prescription bottles in hand. Through her collaborative and thorough approach, she confirmed the correct dose, preventing a potential medication error and reinforcing the patient’s trust in the care process.

Lyn Bartley 

Lyn Bartley, ED technician, demonstrated exceptional attention to detail and adherence to high-reliability principles while changing a behavioral health patient into scrubs. Bartley noticed a waist tie in the scrubs, which is not typical for behavioral health patient scrubs because the tie can be a safety hazard. Bartley immediately stopped the line and removed the pants from the patients scrub supply, preventing future risks.

Sarah Bodin, RN 

Sarah Bodin, RN, Heart and Vascular Center, was recognized for attention to detail and 200% accountability. When reviewing a patient’s telemetry rhythm, she noted a potential discrepancy between the workup and what she was seeing with the patient’s pacemaker. Bodin requested a cross check and escalated her concerns. The patient received prompt treatment and the pacemaker was adjusted for appropriate capture. 

Nicole Gomez, MD, and Zachary Kravetz, MD 

Pediatric residents Zachary Kravetz, MD, and Nicole Gomez, MD, were recognized for practicing a questioning attitude and speaking up for safety with 200% accountability. After a patient was admitted with a diagnosis from an outside hospital, she became febrile and experienced additional symptoms. Suspecting the original diagnosis was inaccurate and after an ultrasound was inconclusive, the doctors continued to push for additional workups and an abdominal CT scan, which showed a perforated appendix.
