PGY2 Ambulatory Care

The Ambulatory Care Pharmacy Residency Program trains pharmacists to develop and deliver patient-centered comprehensive medication management in collaboration with multidisciplinary healthcare teams in primary care and specialty clinics. Residents will also have exposure to ambulatory care administration, teaching and population health management. This enables graduates to enter into numerous ambulatory care pharmacist career paths.

The PGY2 year consists of a series of learning experiences that span a 52-week period.  Specific learning experiences and goals will be accomplished in discrete blocks of time, while others will occur longitudinally throughout the duration of the residency. Residency time will be generally structured as follows:

8. Winchester Chest Pulmonary Clinic 4 weeks*

*Learning Experiences will be 3 days per week.

Longitudinal Experiences

  1. Residency project(s): 
    - Complete a quality improvement and cost savings project
    - Develop a business plan for a new ambulatory care service
    - Submit research project for presentation at ASHP Summer Meeting
    - Prepare at least one manuscript suitable for publication
  2. Education and opportunities to precept:
    - Nursing and physician in-services
    - Clinical case conference presentation
    - Continuing education presentations
    - Peer-reviewed journal publications
    - Co-precept PharmD students and PGY1 pharmacy residents
    - Journal club presentations
    - Didactic lectures
    - Teaching certificate program (optional) 
  3. Staffing*: 
    - Weekday staffing: daytime coverage of ambulatory care clinic shifts (up to 46 shifts)
    - Clinic pharmacist in-basket coverage (schedule to be determined)
  4. Administrative reports/responsibilities:
    - Supervise PGY1 residents and pharmacy students
  5. Ambulatory Care Administration and Health System Clinical Leadership
    - One day per week longitudinally after immersion period has completed
    *Any shift including night shift may be filled at discretion of residency director

PGY2 Ambulatory Care Requirements for Successful Graduation:

  1. Educational checklist complete
    a. Pharmacy Grand Rounds
    b. Clinical Case Conference
    c. Nursing or clinic staff in-services (2)
    d. Physician or pharmacist in-service or newsletter article (2)
    e. Community service (4 events)
    f. Ambulatory protocol (workflow, scope of practice, collaborative practice agreement, or clinical practice protocol)
    g. Ambulatory service business plan
    h. Cost-management project
    i. Manuscript (see below)
    j. Submit research project for presentation at national or regional meeting 
  2. Portfolio complete (Completed documents for each checklist item uploaded to resident portfolio)
  3. Cost management and longitudinal project completed 
  4. Achieved at least 80% of ASHP residency objectives and 100% of objectives for Competency R1 Patient Care (preceptor marked as "achieved" on at least two occasions or other documentation demonstrating achievement of the objective as evaluated by the RPC/RPD)
  5. Manuscript suitable for publication (complete manuscript incorporating at least two rounds of preceptor feedback)
  6. Compliant with all pharmacist mandatory requirements (e.g. current pharmacist license in good standing on file, completion of all mandatory training)

The residency structure of learning is subject to change based on preceptor availability and the discretion of the residency program director and coordinator. An individual resident plan and calendar will be developed for each resident. This residency site agrees that no person at this site will solicit, accept, or use any ranking-related information from any residency applicant.
