Yale New Haven Health participates with most major managed care and governmental health insurance plans to make using our facilities and services as convenient as possible. Participation is not a guarantee of coverage and you may be responsible for deductible, coinsurance and / or copayment. Please review your health insurance policy documents, consult with your employee benefit plan administrator or your insurance plan directly to verify coverage, benefit levels, and referral or authorizations requirements specific to your health plan. Please always provide proof of insurance coverage at each visit.

We remind our patients that they are ultimately responsible for their bill. An insurance authorization is not a guarantee of payment. We suggest you contact your insurance company with questions pertaining to an insurance denial.

"Hospital-Based Outpatient" or "Provider Based" refers to a location, either within the hospital’s main campus or at an off-campus facility, that is owned, operated, and fully integrated with the main hospital. This is a national model of practice for large, integrated delivery systems involved in patient care. For medical services provided to you at these locations we charge a separate hospital or facility fee that is in addition to the professional fee charged by the physician(s) or other medical professionals that treat you. At a hospital-based outpatient facility, your potential financial liability (the amount you may be responsible to pay) may be greater than if the professional medical services were not provided by a hospital-based outpatient facility. Depending on your insurance coverage, you may have certain out-of-pocket expenses (e.g., co-pay, coinsurance, deductible), that you would not incur at a private physician office.

Please check your statement dates to ensure sufficient time has passed between when the payment was made and when the bill was issued. Yale New Haven Health understands that it can be difficult for some patients to afford paying their hospital bills. That is why we have a variety of financial assistance programs designed to help. Patients are required to complete a financial assistance application and provide requested documents to verify financial need. Because we do not know the exact type and extent of services that you will receive when we distribute the Notice to Patients Regarding Billing and Insurance Liabilities the range quoted in the letter is an estimate only. If you have insurance with high deductible or other out-of-pocket liabilities, your insurance company does not provide us with your current annual progress toward satisfying those obligations. Because of this, we cannot determine whether your insurance company will hold you responsible for a certain percentage of your bill, or the entire amount. Our estimate is based on the best information available to us, which is historical data based on other visits to our facilities and typical and average insurance coverage.

In accordance with Connecticut Public Act 14-145 An Act Concerning Fees Charged For Services Provided At Hospital-Based Facilities, which is effective October 1, 2014, hospital facilities are required to inform you when you are being seen for medical services at a hospital-based outpatient facility. You may have received this letter from us in the mail prior to your visit or you may have been handed this letter at your visit. The Notice to Patients Regarding Billing and Insurance Liabilities letter serves to inform you that you are being seen at a Yale New Haven Health hospital and provide you with an estimate of your out-of-pocket expense for your visit based on estimates of our charges and your insurance coverage based on typical visits to the facility you are visiting.

The bills are for professional services rendered by providers in diagnosing and interpreting test results while you were a patient with Yale New Haven Health. Pathologists, radiologists, cardiologists, and other specialists perform these services and submit separate bills. If you have questions about these bills, please call the number printed on the statement you received from the provider.

One or more of the following may apply: the service you received was not covered under your insurance plan; you may not have provided the correct insurance information at the time of service; the service you received was from a physician outside your insurance plan network; you were not covered by your insurance plan at time of service; your primary care physician did not process a referral for the services or an authorization was not obtained prior to the services being rendered. We suggest you contact your insurance company with specific questions pertaining to an insurance denial.

Most insurance plans require you to pay a copayment, deductible and / or co-insurance. In addition, you may be responsible for non-covered services. We suggest you contact your insurance company with specific questions regarding your financial liability.

Information gathered from patient registration is stored in our computer system. We retrieve this information each time the patient returns for services and we ask the patient to verify that the information is current and accurate. Medicare requires that specific questions be asked at each visit to determine whether Medicare or another payor is primary. Your assistance in verifying the information is always appreciated.

The Emergency department is staffed 24 hours per day, and fees are based on the costs associated with being prepared for emergency trauma at any time of day or night. There are a minimum of two charges associated with every emergency department visit; a professional charge and a facilities charge. The exact professional and facilities charge is determined following the visit by reviewing the case severity. Non-emergency visits should be done at your physician’s office or in a clinic setting to keep costs down.

Yes. Inpatient Services, Outpatient Services, and all Emergency Department Services will be billed by Yale New Haven Health for all insurance companies, including Medicare, providing we have the insurance information on file. It is important that you provide accurate and complete demographic and insurance information at the time of registration. Yale New Haven Health will submit bills to your insurance company and will do everything possible to advance your claim. However, it may become necessary for you to contact your insurance company or supply additional information to expedite payment.
