Following Up

Once you are home, we will follow up with you to see how your recovery is going or if there is anything we can do to improve the patient experience at Yale New Haven Hospital.

Follow-up Phone Calls

You may receive a follow-up phone call a few days after discharge to see how you are doing, if you are clear about your discharge instructions and if you have any comments or suggestions about the service you received while you were in the hospital.

Patient Satisfaction Surveys

After discharge, you may be mailed a patient satisfaction survey. Please complete and return it. Your answers and comments will help us make improvements or recognize staff who provided exceptional care.

Giving Back

Grateful patients and family members are often interested in ways to help YNHH recognize the excellent care they or a loved one received. Some are interested in:

  • Giving their time: Call Volunteer Services at 203-688-2297 to find out about the volunteer roles available.
  • Making a financial contribution: To find out more about making a donation, call the Development Office at 203-688-YNHH (9644), visit www.givetoynhh.org.
  • Serving as an advisor: The YNHH Patient and Family Advisor Program engages patients and family members with experience at the hospital who want to help improve the patient experience and promote patient- and family-centered care. For a brochure or more information, call 203-688-8302.
