SLA 5 earns Capacity Champions Cup

Congratulations to staff with Medicine (SLA 5), who recently received Yale New Haven Hospital’s Capacity Champions Cup. The award recognizes units that meet their discharge target the most days in the previous month. It is part of Yale New Haven Health’s efforts to improve patient throughput by ensuring that patients who are clinically ready to leave the acute-care setting experience a safe and timely discharge. SLA 5 leaders and staff attributed their success to effective 9 am huddle discussions and multidisciplinary teamwork, including collaboration among Care Management, Nursing, physicians and advanced practice providers.
