A biometric screening can help identify possible health risks and is required to earn credit through KYN Plus. You may have your biometric screening during a KYN onsite screening event, as part of a preventive visit with your primary care or ob/gyn physician, or at a participating CVS Caremark Pharmacy. Onsite screenings will be available at YNHHS locations throughout the summer. To schedule an onsite screening, visit the Virgin Pulse portal through Infor, Employee Programs.
If you choose to have your screening offsite, download the documentation form from the Virgin Pulse portal to bring to your offsite screening. Completed documentation must be uploaded to Virgin Pulse by Friday, Sept. 1, to receive KYN credit. Earned credit for a biometric screening is $300.
Complete Personal Choice Activities to support your health and well-being and earn additional medical plan premium credit toward the $500 maximum. Credit amounts vary by activity. Please note that earned credit remains “locked” until you receive a biometric screening. Visit the Virgin Pulse portal through Infor, Employee Programs, to review and select Personal Choice Activities.
