Yale New Haven Health System’s equipment formulary is the go-to resource for ordering vetted and approved materials and equipment from the health system’s contracted vendors based on clinical, organizational and contractual needs.
To register for formulary access, email [email protected]. Once registered, employees can access the formulary on the employee intranet. Go to “Departments and Communities” and “Supply Chain” and choose “Equipment formulary” from the right menu. Use your network login credentials to access the formulary.
Launched last year, the equipment formulary is designed to make ordering easier. It provides product information, pictures and technical specifications, along with vendor contact information to request price quotes and ask questions. The formulary also helps YNHHS continue standardizing clinical and non-clinical supplies and equipment and control costs.
