Officer Kerry Deegan, Protective Services, was recognized for attention to detail, practicing a questioning attitude and speaking up for safety. While completing routine rounding at Yale New Haven Psychiatric Hospital, he noticed a potential hazard in the courtyard where staff often take patients for fresh air and physical activity. He escalated his concerns to service line leaders, and a search of the area revealed additional hazards. All were removed, preventing potential harm to patients and staff.

Aaron Miller, chaplain, Spiritual Care, was recognized for attention to detail, practicing a questioning attitude and speaking up for safety. While visiting a patient the day before a scheduled surgery, he discovered the patient was unaware of the pending procedure. Miller confirmed the patient's name and medical record number and asked the nurse to investigate. The nurse confirmed that there was no mention of surgery for that patient and contacted the cardiologist, who verified that the patient was not scheduled for surgery The incident revealed an issue with the alphabetical listing of patients, which was corrected.
