Since 1984, Yale New Haven Hospital has been leading the way in delivering lifesaving care, including heart transplantation, for patients with advanced heart failure.

Our Yale New Haven Hospital Heart and Vascular Center (HVC) uses an integrated team approach that reflects collaboration among cardiac surgery, cardiology and our advanced heart failure program. Patients benefit from a full spectrum of expertise, from advanced medical management and heart support devices to transplantation and long term follow-up. Our specialists develop personalized treatment plans that reflect the latest evidence, innovative therapies, and each patient’s unique clinical needs.

The Heart and Vascular Center offers advanced heart failure and transplant care for children, adolescents, and adults. We are proud to be the only hospital in Connecticut offering pediatric heart transplants.
