Please follow these simple steps to submit a request as a prospective patient. Please be aware that you or your physician will need to submit your medical records, translated in English, as part of this process.

Call us at +001-203-495-9253 or complete the brief Patient Information Request Form to provide us with information about yourself and the medical services you are seeking. Our call center is open 24 hours a day, every day of the week. Interpreters are available for more than 200 languages.
An International Medicine Program Coordinator will contact you within 1-2 business days after the Patient Information Request Form is completed. You will receive instructions on how to submit your medical records. Your records must be translated into English.
Within 7 business days of receiving all medical records, our physicians will review your information. If we determine that the International Medicine Program is appropriate for your care, we will provide an estimate for hospital and physician expenses.
A full financial guide will also be provided. It will outline all payment conditions and financial requirements that must be met by the patient prior to receiving treatment.
Payment for all charges is required prior to scheduling any medical appointments or assistance with travel arrangements. Your International Medicine Program Coordinator will work with you to process your payment. Once full payment is received and we determine that our program is appropriate for your care, you will officially become a patient of the International Medicine Program, collaboration between Yale Medicine and Yale New Haven Hospital.
