Know Your Numbers — helping you thrive

Health screenings, expanded personal choice activities, highlight this year’s program

Yale New Haven Health’s Know Your Numbers (KYN) Plus program gives you the power to make decisions that impact your health. With the COVID-19 pandemic highlighting the importance of well-being, a biometric screening that provides a snapshot of your physical health (your “numbers”) is an important piece of knowing where you stand after a year like no other. 

“The KYN Plus program doesn’t stop at biometric screening,” said Kevin Myatt, senior vice president and chief human resources officer, YNHHS. “As part of Thrive, the new hub for all YNHHS employee well-being resources, KYN Plus now includes an expanded list of activities – extending into areas such as emotional, social and financial wellness – to help bring your health and well-being full circle.”

YNHHS benefits-eligible employees hired on or before Aug. 30, 2021, can earn up to $500 credit toward their 2022 YNHHS medical benefits premium by participating in KYN Plus. While employees can use the many Thrive resources year-round to enhance their overall well-being, the KYN Plus component runs through Sept. 15.

Step 1: Enroll in the Virgin Pulse Employee Well-being Portal

Employees who choose to participate in KYN Plus must enroll in the portal. Employees who already enrolled do not need to re-enroll. Access the portal from the Resources section of the employee intranet. YNHHS benefits-eligible employees must also complete steps 2 and 3 to earn up to $500 credit toward their 2022 YNHHS medical benefits premium. 

Step 2: Receive a biometric (health) screening

Biometric screenings are brief, in-person health assessments that provide a picture of your overall health and alert you to any possible risks. Screenings provide information on:

  • Total cholesterol, HDL (good cholesterol), LDL (bad cholesterol), triglycerides and blood glucose
  • Blood pressure
  • Height, weight and body mass index (BMI)
  • Waist and hip measurements
  • New: A1C, for employees who indicate they have diabetes during their pre-screening questionnaire

Starting the week of May 17, YNHHS will conduct onsite screenings at all delivery networks. Appointments are required; to schedule, visit the Action Dashboard (Employee Self Service) on Infor. Alternatively, employees may receive a biometric screening from their healthcare provider or at a CVS Caremark location. YNHHS benefits-eligible employees must upload documentation of their screening to Virgin Pulse to receive their KYN credit. 

Completing the biometric screening earns $200 credit (toward the possible $500 total credit). Employees must complete a health screening to earn additional credit through KYN Plus.

Step 3: Participate in personal choice activities

All employees can work on their well-being with Thrive resources any time throughout the year. To access resources, log into Virgin Pulse and select the Benefits tab from the top menu.

The table on this page includes an expanded list of personal choice activities available to KYN Plus participants through Thrive. YNHHS benefits-eligible employees can earn a medical premium credit for these activities, as indicated in the Credit Value column. Employees may participate in as many personal choice activities as desired, but the maximum credit they may earn is $500. Any credit earned by completing KYN personal choice activities will remain “locked” until the employee completes a health screening.

For details on personal choice activities and to track earned credits, use the Rewards tab in Virgin Pulse. Eligible activities completed between Oct. 1, 2020 and Sept. 15, 2021 qualify for credit. 
 
All KYN Plus activities must be completed by Sept. 15, 2021. Employees will receive a KYN brochure in the mail with details. For more information, visit the employee intranet, call HR Connect at 1-844-543-2147 (option 1) or speak to your manager.
