Simona Jakab, MD, attending hepatologist and associate professor of medicine (digestive diseases), Yale School of Medicine, was recognized for attention to detail and practicing a questioning attitude – which likely saved the life of one of her patients. As part of the “Liver Home” remote patient monitoring program, the patient records his own blood pressure and weight. When he did not arrive for an appointment and no data were recorded in Epic, Dr. Jakab became concerned. She called for a wellness check and the patient was found unwell in his home. Thanks to Dr. Jakab’s use of CHAMP behaviors, the patient received immediate, lifesaving care.

Susan Fitzgerald, RN, Verdi 4 East, was recognized for attention to detail, practicing a questioning attitude and escalating concerns. While assessing a patient in the PACU, she noticed that the patient's left leg looked shortened and promptly notified the manager. Further examination determined that the patient's hip was dislocated due to a previous medical condition. The patient was taken back to surgery that evening.
