Debrief highlights quick response to serious safety event

The 55 Park St. auditorium was filled beyond capacity at the quarterly serious safety event (SSE) debrief on May 30. The debrief focused on the quick response after an event involving a patient who was on the way to the MRI suite. Members of the root-cause analysis team described how they rallied to create a systemwide remediation plan that was implemented only six days after the event. Senior leaders joined attendees from a variety of departments and roles at the debriefing. SSE debriefs are designed to promote transparency around events of harm and provide a forum to share lessons learned across the organization and corrective actions taken to improve patient safety.
