Bryan Townsend, RN; Joshua Bia, MD; and Jorge Zaldivar, MD, were recognized for attention to detail and escalating concerns when a patient arrived at the Emergency Department after a fainting episode. When EMS reported that the patient had fallen before the 911 call, Dr. Zaldivar contacted people at the place where the patient had been to find out the timing of symptoms and the fall. The ED team determined that the patient’s symptoms indicated signs of stroke, and called a stroke alert. Thanks to their quick action and clinical expertise, the patient quickly received the appropriate life-saving care.
Matthew Davis, PharmD, clinical pharmacy specialist, was recognized for attention to detail, practicing a questioning attitude and escalating concerns. When reviewing a patient's microbiology test results, Davis saw bacteria with a resistance profile that was confusing and contradictory to what would normally be expected. He escalated his concerns to the Microbiology Lab. Further investigation showed that a change in the algorithm used to determine resistance had unknowingly occurred in June. Davis’ use of CHAMP behaviors ensured the patient received the appropriate antibiotics; helped prevent a potentially life-threatening outcome; and identified an opportunity for process improvements.
