Clinical Fellows

Clinical Fellow Applicants

This page is intended for clinical fellow applicants. All others should proceed to Medical and Affiliated Staff Applicants page.

All ACGME and NON-ACGME clinical fellows employed by either Yale New Haven Hospital or Yale University must complete a Clinical Fellow Application and be approved before beginning work in any capacity.

Clinical Fellow Application Materials and Instructions

Application request

In order for YNHH to launch clinical fellow applications, the department specific fellowship coordinator will submit incoming clinical fellow information to the YNHH Medical Staff Administration Department.

Once the information is received, an application will be launched to the clinical fellow applicant.

Please note: Clinical fellow applicants may not begin working at YNHH in any capacity until notification is received from Medical Staff Administration. The YNHH Photo Identification office will be notified once approval has been granted.

  • Copy of your CV
    •  Any gaps longer than 1 month will need a full explanation.
    •  Please note that more than 4 years in Medical School within the U.S. will need an explanation.
    •  Ensure your CV is in chronological order and show ALL start and end dates formatted as MM/DD/YYYY).
  • NPI Number – Required prior to submitting the application.
  • ALL Vaccine information/dates and/or titers:
    •   Measles, Mumps, and Rubella (MMR) (2 Doses)
    •   Varicella (Chickenpox) (2 Doses)
    •   Hepatitis B
    •   Tetanus/Diphtheria/Pertussis (Tdap)
    •   Influenza
    •   COVID-19 - Strongly recommended, but not required
    •   Tuberculosis (TB) Screening
      •   Documentation of two negative TB skin tests (i.e. PPD, TST or Mantoux skin test within 1 year of requested start date)
      •   OR
      •   a negative TB blood test (i.e. Quantiferon, T-spot, or BAMT) within six months of your requested start date.
  • Medical Staff Education Training, (Required for ALL)
  • Adult Moderate Sedation (Required ONLY if seeking Moderate Sedation privileges)
  • VISA or other temporary resident documentation if you are not a U.S. Citizen

Adult Moderate Sedation

If you are seeking privileges in moderate sedation, please:

1. View the video, Sedation and Analgesia for non-Anesthesiologists
2. Take the Post-Test. You must pass the test to be eligible for privileges. Note: Be sure to click through the screens to record your score and screenshot the page showing your name, date and test score. Provide this to your credentialing specialist.
3. Provide proof of current Airway Management Training/Certification.
