Clinical Fellow Applicants

All ACGME and NON-ACGME clinical fellows employed by either Yale New Haven Hospital or Yale University must complete a Clinical Fellow Application and be approved before beginning work in any capacity.

In order for YNHH to accurately track applications in process, each fellowship coordinator is required to submit the names of the clinical fellows who will be working in his/her respective department/section by e-mail to Karen Gleeson and Iris Atkinson.  A confirmation e-mail will be sent acknowledging receipt of the notification. If you do not receive a confirmation within five business days, please contact the Medical Staff Administration at 203-688-2615.

Clinical Fellow Application Materials and Instructions

Application request

To apply for appointment as a clinical fellow, applicants must read the documents noted below and complete and submit all of the materials as indicated under Items to return.

Please note: Clinical fellow applicants may not begin working at YNHH in any capacity until notification is received from Medical Staff Administration. The YNHH Photo Identification office will be notified once approval has been granted.

Applications and supporting documents should be emailed to Iris Atkinson.  You must confirm receipt in order for the application to be processed.  Alternatively, hard copies of the materials can be mailed or hand-delivered to:

Complete, signed, dated clinical fellow application. Note: The bottom half of the form entitled "Recommendation for Appointment to the Clinical Fellow Staff" must be completed by the appropriate department/section chief.

CSR declaration

DEA declaration

Immunization Documentation Form, including evidence of PPD within the past 12 months 

Copy of curriculum vitae in month/year format, with explanation of any work history gaps of 1 month or more.

Adult Moderate Sedation* 


If you are seeking privileges in moderate sedation, please:

  1. View the video, Sedation and Analgesia for non-Anesthesiologists
  2. Take the Post-Test. You must pass the test to be eligible for privileges.
  3. Provide proof of airway management in the form of a ACLS or PALS certificate that is less than four years old.    

not applicable for physician applicants trained in anesthesiology, emergency medicine or critical care medicine.
