The mission of the Department of Pharmacy is to provide high value, patient-centered care. We will promote best practices that drive safe, effective, and timely medication therapy through all transitions of care. We will cultivate collaboration, innovation, education and career growth through employee engagement and practice advancement initiatives.
Through participation in diverse learning experiences, a pharmacist completing this program shall gain the skills and attributes necessary to positively affect patient outcomes in a variety of settings, to participate in the multidisciplinary provision of healthcare across the continuum, and to effectively manage their individual professional practice. The resident will synthesize knowledge and experience in the formulation of patient-specific, cost-effective pharmacotherapeutic plans. The resident will demonstrate effective verbal and written communication skills in education of peers, other healthcare professionals, patients and members of the community about drug and health related topics. The resident will exhibit leadership, prioritization skills, and problem solving ability in their daily practice and will demonstrate a commitment to life-long learning. The resident will gain the tools necessary to adapt their practice in response to a continually changing healthcare environment. Residents will be active participants in departmental planning, budgetary preparation, resource management, quality improvement processes, medication use evaluation and new program development.
Yale New Haven Health has four PGY1 and twelve PGY2 Residency programs. You can follow the links here to learn more about the each program.
PGY1 Pharmacy Program Purpose: PGY1 pharmacy residency programs build on Doctor of Pharmacy (Pharm.D.) education and outcomes to contribute to the development of clinical pharmacists responsible for medication-related care of patients with a wide range of conditions, eligible for board certification, and eligible for postgraduate year two (PGY2) pharmacy residency training.
PGY1 Community‐Based Pharmacy Residency Program Purpose: To build upon the Doctor of Pharmacy (PharmD) education and outcomes to develop community‐based pharmacist practitioners with diverse patient care, leadership, and education skills who are eligible to pursue advanced training opportunities including postgraduate year two (PGY2) residencies and professional certifications.
PGY2 Pharmacy Program Purpose: PGY2 pharmacy residency programs build on Doctor of Pharmacy (Pharm.D.) education and PGY1 pharmacy residency programs to contribute to the development of clinical pharmacists in specialized areas of practice. PGY2 residencies provide residents with opportunities to function independently as practitioners by conceptualizing and integrating accumulated experience and knowledge and incorporating both into the provision of patient care or other advanced practice settings. Residents who successfully complete an accredited PGY2 pharmacy residency are prepared for advanced patient care, academic, or other specialized positions, along with board certification, if available.
